Ninety percent of the men on one testosterone-blocking shot got hot flashes. The men on the other shot had more heart trouble. Both drugs shut the hormone down about equally, which is the point: the two most common ways to starve a prostate tumor of testosterone are not interchangeable, and a new trial spells out what you trade when you pick one.
Prostate cancer feeds on testosterone. The standard way to take that fuel away is androgen deprivation therapy, and the workhorse drugs are LHRH agonists, peptide copies of the natural brain hormone that tells the testes to make testosterone. Give the body a steady flood of that signal instead of its normal pulse and the pituitary stops listening, testosterone crashes, and the tumor loses its supply. Two of these peptides have been in use for decades: leuprolide ↗, sold as Lupron and Eligard, and goserelin, sold as Zoladex. Clinicians tend to treat them as the same tool. A trial out of Brazil says they are not.
Writing in the Canadian Journal of Urology ↗, a group led from the Hospital do Câncer de Muriaé and the Santa Casa BH medical school ran a prospective, randomized, controlled study in 174 men with prostate cancer, following them through a full year of treatment. They compared leuprolide, given at two depot doses (22.5 and 45 mg), against goserelin at 10.8 mg, and tracked not just whether the drugs worked but what they did to the rest of the body: PSA and testosterone, blood lipids and blood sugar, side effects, and a set of cancer-related genes.
On the job they were hired for, both worked. Every group saw significant drops in PSA, the blood protein that tracks prostate-cancer activity, and in testosterone. Leuprolide at 22.5 mg produced the most pronounced PSA reduction of the lot. Goserelin was shakier out of the gate, with more variable testosterone in the early months, meaning the suppression it delivered was less steady before it settled.
The divergence was in the cost. Hot flashes, the classic penalty of shutting down sex hormones, hit 90 percent of the men on leuprolide 22.5 mg, and that group also gained more weight. Goserelin traded a different currency: cardiovascular events were more common in that arm, and it carried the hormonal instability noted above. So the choice is not potency versus nothing. It is a strong, steady PSA drop that comes with sweats and weight on one side, against a rockier hormonal course with more heart risk on the other.
The trial also read four cancer-linked genes in these men. TP53, the most famous tumor-suppressor gene, was altered in about one in five (20.4 percent), followed by BRCA2 (7.4 percent) and ATM (6.5 percent). Those mutations did not change how well the drugs suppressed the cancer, though on a sample this size an absence of difference proves very little. The one thread the authors pull is that BRCA2 variants tracked with lipid metabolism, a hint that a patient's genetics might shape the metabolic side of ADT even when it does not shape the anti-cancer side.
The honest limits are worth stating plainly. This is 174 men at centers in one country, followed for a year, with PSA and testosterone as the readouts. Those are the standard surrogates for whether ADT is doing its job, but they are not survival, and a year is not long enough to see how the cardiovascular and metabolic costs actually land over the decade a man may spend on these drugs. The genetic arm is small and exploratory. What the trial does well is puncture the assumption that two generic peptides with the same mechanism are the same clinical decision.
The mechanism itself is a recurring character on peptidemodel. Leuprolide sits on the GnRH receptor ↗, the same switch behind the shift toward non-hormonal approaches we covered when the FDA cleared a peptide that clears endometriosis lesions instead of forcing a chemical menopause ↗. That drug's pitch was escaping exactly the tradeoff this trial measures. Goserelin, oddly for a peptide this widely used, has no card on the platform yet. The prostate-cancer story here is about anticancer ↗ hormone control, and the tradeoff it documents is the reason the field keeps hunting for a fourth option.
